Dr. John Frattarelli is a nationally and internationally recognized Reproductive Endocrinologist with over 30 years of experience in reproductive medicine and infertility care. He is the Founder, CEO, and Medical, Practice, and Laboratory Director of the Fertility Institute of Hawaii, one of the most advanced and trusted fertility centers in the Pacific region. Under his leadership, the Institute has earned a reputation for combining the latest reproductive technology with personalized, compassionate, and innovative patient care, helping thousands of patients from around the world achieve their dream of parenthood.

Dr. Frattarelli is double board-certified in Obstetrics and Gynecology and Reproductive Endocrinology and Infertility. Additionally, he is among the few physicians worldwide to hold certification as a High-Complexity Clinical Laboratory Director (HCLD), which allows him to oversee the clinical and laboratory aspects of all fertility procedures in the In Vitro Fertilization (IVF) laboratory. This rare dual role ensures his patients receive the most integrated and comprehensive fertility care available.

Education and Training

Dr. Frattarelli graduated magna cum laude from Vanderbilt University with a double major in Classical Studies and General Biology. He earned his medical degree from the University of Tennessee College of Medicine in Memphis. He completed his internship and residency in Obstetrics and Gynecology at Walter Reed Army Medical Center in Washington, D.C., and then finished his fellowship in Reproductive Endocrinology and Infertility at the renowned National Institutes of Health (NIH) in Bethesda, Maryland.

Military Service and Early Career

Dr. Frattarelli served as a physician in the U.S. Army for over a decade, departing with the rank of Lieutenant Colonel. During his military career, he was the Chief of the Division of Reproductive Endocrinology at Tripler Army Medical Center (TAMC) in Honolulu, Hawaii, where he also served as the Director of the TAMC IVF Institute for six years. His service combined leadership in clinical medicine, resident education, and research programs, earning him multiple military commendations.

Dr. Frattarelli has been consistently recognized as a “Top Doctor” by Castle Connolly and “Best Doctor” by Honolulu Magazine every year since 2009.

After finishing his military service in 2006, Dr. Frattarelli became the Clinical Research Director at Reproductive Medicine Associates (RMA) of New Jersey, one of the country’s most prominent IVF centers, where his research team received multiple national awards and presented internationally. In 2008, Dr. Frattarelli returned to Hawaii to establish Advanced Reproductive Medicine & Gynecology of Hawaii, Inc., dba: Fertility Institute of Hawaii.

Dr. Frattarelli has held faculty appointments at several academic institutions, including Assistant Clinical Professor at the University of Hawaii’s John A. Burns School of Medicine and Associate Professor at Rutgers University in NJ. He has trained dozens of medical students and residents and has served in leadership roles in medical education, such as the Council on Resident Education in Obstetrics and Gynecology (CREOG).

Dr. Frattarelli has held leadership positions in national and international organizations such as the American Society for Reproductive Medicine (ASRM), the Society for Reproductive Endocrinology and Infertility (SREI), the International Society of Reproductive Genetics (ISRG), and the Pacific Coast Reproductive Society (PCRS), where he served as President from 2021–2022.

Dr. Frattarelli is a leading researcher in the field of reproductive medicine with over 200 presentations and peer-reviewed publications both nationally and internationally. He has served as Principal Investigator on more than 20 clinical trials focusing on IVF success, ovarian stimulation, embryo quality, and preimplantation genetic testing. His studies have helped shape clinical protocols used in fertility clinics worldwide, and his research continues to influence best practices in IVF and reproductive endocrinology.

A Personal Connection to IVF

Despite his many accomplishments, what truly distinguishes Dr. Frattarelli is his deep empathy for patients because he’s walked the IVF journey himself. More than just a REI specialist, Dr. Frattarelli is also a former fertility patient. His four children were conceived through IVF, ICSI, PGT-A, and cryopreservation. He used surrogacy for his youngest child. This personal and comprehensive experience with fertility medicine to build his family further enhances his uniquely personal and empathetic perspective on the journey many of his patients face. He understands the emotional, physical, and financial challenges of fertility treatment and is dedicated to making the process as supportive and successful as possible.
